- get you to the right person or team as quickly as we can
- let you know what the different teams and departments do
- work well together to support the people of Devon
Consent to contact us
If someone you are working with needs to contact us, it is preferred that you ask them to contact us directly. If you do need to refer them or give updates before they contact us themselves, you must ensure you have their consent to do so.
Referrals to Adult Social Care
If you are required to refer someone, please use the online referral form for professionals.
Please ensure:
- you gain consent from the individual before completing a referral form on their behalf
- there is enough information contained within the referral form to enable us to understand what the person requires – we will not be able to progress referrals with limited information
If you are unable to use the online referral form and would prefer to speak to someone, please continue to do so by contacting your local CDP team on the numbers below. Please note you will need to have the person with you when making the call to ensure consent.

Giving or receiving an update about a person you are working with
Please complete the relevant online referral form (available above) and select the option to give an update about a person you are working with or call the appropriate team.
If the person is being supported by a community team, please ensure you contact that team for an update.
Finance or remittance queries for providers
Remittance advices or breakdowns are now available electronically. If you haven’t been set up for emailed remittances yet, we need a secure shared mailbox to enable this new process.
For finance-related, remittance breakdown or email queries, please complete this Finance and remittance queries (Adult Social Care) form – details will also be available on your remittance advices for Adult Residential and Personal Allowance payments.
