5a) Understand the moves and changes in Educational settings.I can experience an effective and positive transition from one setting to another and within a setting.  I have an awareness of the additional support available in my next educational setting.    I know about different studies and courses and what I need to achieve to achieve my aspirations.I have the tools, knowledge, and skills to self-advocate.  I have an understanding of the support mechanisms available to manage transition beyond secondary school.
5b) Understand my own access needs      I can express my individual preferences to improve my accessibility: I know about reasonable adjustments and the requirement of the setting to meet these.I have an understanding the of the different access options available, eg interpreters, notetakers, lip-speakers and speech to text apps, ToD support.  I know that my access needs may change over time and what the process is for requesting any changes.      I have a good understanding of the  access arrangements that are available and I need for assessments and exams.    I am able to have discussions about my career pathway and next steps.  
5c) Moving to adult NHS services  NANAI understand the differences between paediatric and adult services and when this will occur.  I am able to advocate communication preferences for any appointments.  I feel supported in understanding possible best care pathway options
