4. I wish to change from our existing cover for Absence – Schools Mutual Fund Scheme to the level indicated on this form. I understand the premium is based on January numbers of staff pay factor rates as confirmed by the Payroll Team or by Academy schools themselves.*
5. Teaching Staff Option Chosen:*
Choose your coverage option for teaching staff, based on number of waiting days before claim is paid and reimbursement rate, further information can be found in the Mutual Fund Calculator.
6. Support Cover*
Please select whether you would like to add optional support staff cover and the level of cover required.
