This form is to be completed each year by non-Devon maintained schools, such as academies or free schools to allow the Mutual Fund Premium to be calculated and charged. Please be aware that the data entered on this form is accepted in good faith.

This form will need to be completed on or before 31 March each year, from which the Mutual Fund calculator will be updated accordingly.
