By completing this form you agree that the school Mutual Fund membership will terminate on the 31st March of the current financial year. From 1st April the school will no longer be covered under the scheme, therefore we recommend that you enter into an arrangement with an external provider or risk meeting the cost of cover with your own delegated budget.
