  1. I confirm that I meet the qualifying criteria listed below, for the provision of an on-street disabled parking bay.
  2. I hold a current Blue Badge which is valid for more than 3 months from the date of this declaration.
  3. I am the driver or regular passenger of the vehicle for which the bay is required.
  4. The vehicle is based at my property, which the driver is also a resident.
  5. There is no off-street parking at or near my place of residence.
  6. I understand that an on-street disabled parking bay cannot be supplied exclusively for my use and that any bay provided may be used by any Blue Badge holder.
  7. I understand that I must display a valid Blue Badge at all times while parked in the bay.
  8. I understand that misuse of the bay may lead to its removal.
  9. I understand that making a false declaration on any of the criteria above, may lead to my application being rejected or the disabled bay being removed and may impact future applications.
  10. I confirm that I shall inform Devon County Council or any changes to my circumstances (i.e. change of address or ceasing to drive) that would mean that the bay would be unused, so that the bay may be removed and the parking stock returned to general use. Failure to inform Devon County Council that the disabled bay is no longer required may lead to a delay in future applications.
  11. If the bay is not to be placed outside of my property, I confirm that I agree to the proposed location and that it will not cause dispute with my neighbours (in some cases it can be helpful to discuss proposals with neighbours).
