Social workers aim to improve peoples’ lives by helping with social and relationship difficulties, promoting rights and wellbeing. Social workers protect children and adults with support needs from harm. From helping keep a family under pressure together to supporting someone with mental health problems, social workers engage individuals, couples, families, groups and communities.

Social workers are at the heart of what we do. In Devon, we’re fully committed to providing all the support, training and development our Social Workers need in order to continue to improve their professional practice. This includes regular supervision, appraisals, and learning and development opportunities.

Community Teams:

Our vibrant and friendly community teams cover both very rural and urban areas with posts co-located in community hospitals, there are excellent development opportunities for Social Workers who wish to progress their career. The community teams work with adults of working age and older adults who may need support because of physical or learning disabilities.

Hospital Discharge Teams:

Our hospital discharge teams support adults with physical disabilities and older people. These teams complement the work of the community health and social care teams, providing a comprehensive approach to hospital discharge (acute and in some instances community hospitals).

Social Workers work closely with a multidisciplinary team to provide a strong social care presence.

Transitions Teams:

Our Transition Teams works closely with a range of colleagues and professionals to ensure the smooth transition of services provided to children and young people aged 14 and above, as they move between children’s and adults’ services provision.

Peripatetic Team:

Our Peripatetic Team undertake rotations in various teams across the Devon County Council footprint (community, hospital, or specialist team).

Autism & ADHD Team:

The Autism & ADHD Team carry out social care assessments, commissioning, monitoring and case management for individuals with Autistic Spectrum Disorders (without additional Learning Disabilities / intellectual impairments) and/or Attention Deficit Hyperactivity Disorder (ADHD) or related conditions with complex social care needs in Devon.

Sensory Team:

The Devon Sensory team operates across the whole Devon County Council area. We offer assessment in the context of Care Act, support and training to people whose lives are affected by significant sight or hearing issues.

Specialist Placement Team:

The Specialist Placement Team are a social care team who work with adults with Learning Disabilities/Autism and ADHD who are placed outside of Devon. They work with people with a variety of different support needs who live within a multitude of different settings, from supported living, community homes, residential and supporting people who are leaving hospital.

Adult Mental Health Teams:

Devon County Council also have assigned Social Workers in Devon Partnership Trust delivering the statutory functions of the Local Authority within Adult mental health.

We offer training and development opportunities for all staff and clear career pathways for Social Workers and managers. We create a safe space for reflection and are committed to growing a positive learning environment with a clear commitment to developing Newly Qualified and Student Social Workers.
