S11 data collection form
The sample S11 data collection on admission to school form available to schools has been updated. Information on changes to the form are listed on the first page. Please note this is a sample for which captures information required for the school census and other information that might be relevant to schools. It is not a mandatory form, schools can produce their own data collection form or make amendments to the sample form as they consider necessary.
