12 months to January 2021 = 1413 social care reablement services started.

In response to questions 8-9, please see our response to questions 2 and 3; we therefore do not hold this information.

10. What KPIs (Key performance Indicators) are used for the service/ the service is measured against?

Nationally, we report 3 indicators, but due to Covid-19 the recording has been affected which has impacted on the data available.

2B1 – Proportion of older people (65+) who were still at home 91 days after discharge from hospital into rehabilitation/reablement
2B2 – Proportion of older people (65+) who received rehabilitation/reablement after discharge from hospital
2D – The outcome of short term services – sequel to services (no ongoing support, or support of a lower level)

We have a range of internal local indicators that are used to monitor the service on the ground, but the D2A pathways have resulted in data quality issues.

11. What is the performance against these KPIs over the past 12 months?

Due to impact of covid-19 on recording figures provided for 2019/2020
2B1 – Proportion of older people (65+) who were still at home 91 days after discharge from hospital into rehabilitation/reablement – 85.8%
2B2 – Proportion of older people (65+) who received rehabilitation/reablement after discharge from hospital – 1.7
2D – The outcome of short term services – sequel to services (no ongoing support, or support of a lower level) – 80.5%
