1. How many child safeguarding practice reviews the council has carried out in each year since 1 January 2020?
To clarify, it is not Devon County Council who carry out these reviews, but the Devon Children and Families partnership. We do not hold this information by calendar year, but are able to provide it by financial year:

3. Please include whether the case was referred to the national child safeguarding practice review panel.
All multi-agency reviews, where WT2018 criteria has been reached are referred to National Panel via the Serious Incident Notification, which is completed by an officer of the Local Authority after multi-agency discussion/agreement. This happens within 5 working days of the death or serious incident. ‘Rapid Reviews’ are then carried out over 15 working days with National Panel being advised of the outcome. The information in the table above above reflects the number of rapid reviews that led to a full Child Safeguarding Practice Review.
