1. Please can you provide a copy of the form(s) and criteria used for mobility assessments for a Blue Badge. 

Applications are assessed in accordance with the Blue Badge scheme local authority guidance.

2. I would also like to see a copy of the procedure for evaluation of applications.

3. Do you use an external provider for Independent Mobility Assessments and if so who is this?

External provider – Access Independent.

4. Please can you also tell me for the period of January 2023 to December 2023 and also January 2024 to December 2024.
a. The number of administrative staff.

2.09 FTE Admin, 0.33 Manager.

b. Number of assessors within the team.

We don’t have any “Expert Assessors” within the Blue Badge Team, however our 2 Blue Badge Admin Officers carry out an initial assessment of applications and will make a decision where they are satisfied there is sufficient information and evidence to approve an application

Where they are unable to decide based on submitted evidence the application will be passed to Access Independent for assessment with an expert assessor
