4) What plans are in place to bring these placements back into the County Council?

The Specialist placement Team allocate a Senior Social Worker (SSW) to work alongside Nurses in the Individual Patient Placement (IPP) team to progress discharge back to Devon or chosen Community to be near family and friends. Specialist Placement Team (SPT) and IPP attend Care Programme Approach (CPA) meetings managers hearings and Mental Health Tribunals to feedback progression towards discharge plans. Enhanced brokerage is used to identify providers with necessary skills to meet complex needs. Individual Service Design is commissioned by Clinical Commissioning Groups (CCG) to inform person centred planning. The Transforming Care Grant can be access to purchase property to be rented to an individual via Housing Association where and individual requires singleton placement. Once provider and accommodation are identified, transition to new placement progresses. Core groups which include clinicians, SSW, family, support provider and the individual if they wish are convened. The focus of this is to plan in detail the transition and ensure that a robust plan is in place and receiving health and social care teams are fully engaged. The SPT SSW will then continue to monitor for three months to ensure robust handover to local team is complete.

5) How many service users have no identified independent or supported living home placement in the County Council at the present time?

We do not hold this information in an easily reportable format, it would require a manual review of the case files of thousands of service users, each one requiring several minutes, making a total time required in excess of the 18 hours allowed under the Freedom of Information Act 2002 Section 12.

6) How many of the service users require flat access or mobility adaptations to support them?

We may hold this information but it would be included in some client assessments but we would have to manually examine over four thousand of them, even if each one took only ten minutes that would mean a total of nearly 700 hours, well in excess of the 18 hours allowed under the Freedom of Information Act 2000 Section 12

7) How many individuals with learning disabilities between the ages of 16 and 18 are supported by the County Council?

We may hold this information but it would be included in some client assessments but we would have to manually examine over   thousand of them, each one taking   minutes, meaning a total time of   hours, well in excess of the 18 hours allowed under the Freedom of Information Act 2000 Section 12.
