Amended S11/1 template form

Amendments July 2023

• Sex and gender identity amended – the DfE are replacing the ‘gender current’ data item with the new ‘sex’ data item.
• The ‘gender’ data item has been created so schools, children’s homes, local authorities and other institutions can record gender identity for local use, should they wish to.
•  Data protection and compliance weblinks updated.

Please note this form is a sample form which captures information required for the school census and other information that might be relevant to schools. It is not a mandatory form that schools must use, schools can produce their own admission form, or amend the sample form as they consider necessary.
