Building a Devon where people care for and support each other to become and remain independent
We want to help people to live their lives in an independent way that makes sense to them, and for health and care services to understand what living a good, independent life looks and feels like.
We are doing this by gaining a deeper understanding of health and social care in Devon and what using these services really feels like through the experiences of residents and professional staff.
In Northern Devon, community health and social care colleagues are learning about the how the current system works for the people who use our services and the staff who work in the services. They are also learning and developing practice to understand what matters to people so we can support people in the right way and at the right time.
(Short version) By using principles: having a different conversation; making sure we understand what people want and what matters to them; listening to what is important in people's lives to add value; and being a consistent point on contact.

Using a simple tablet, the group were supported by Joe to create their own video about “What do I need to remember to take with me to go out for the day?” which is triggered by a QR code that is scanned in the building in an accessible place.
