Primary Care services generally remain the same for young people when they become adults, but you may find your GP takes more responsibility for your health care.
There may also be some services outside routine appointments, that are not free to access as an adult. An example might be NHS dental services and orthodontics. Check if you’re eligible for help.

We work closely with NHS Devon and our Public Health team to understand the needs of our population, including:
- what services are most valuable in meeting needs
- what can be done to make health services more effective and accessible
- how to identify the gaps in services

Livewell Southwest
Livewell Southwest provides community health services across Plymouth. Their Complex Review and Transitions team works with adults and young people in transition to adulthood who are living with a range of disabilities and long-term conditions. The team typically supports those who may have a combination of learning disabilities, developmental disorders and/or mental health difficulties and the goal is to support them to take greater control over their lives.
