Information about the health services available in Devon to support children and young people who have significant health needs or disabilities

Specialist health services

  • Children and Young People's Continuing Care (CYPCC)

    Children and young people under 18 with complex health needs that can’t be met by GPs, hospitals or NHS community services may be eligible for CYPCC support. A referral from a healthcare professional, usually a Children’s Community Nurse or Paediatrician, is needed to access this.

  • Neurodiversity support

    Children and Family Health Devon (CHFD)’s neurodiversity pathway helps children and young people in Devon access support for conditions such as autism, ADHD, tics and Tourette’s syndrome. Parents and carers can request support directly – a referral is not necessary.

  • NHS Continuing Healthcare (18+)

    Continuing Healthcare (CHC) is an NHS fully funded package of care that some people aged over 18 receive due to their long-term health needs. You will need a referral from a health or care professional to access this service, for example, a doctor or consultant.

  • Children's Hospice South West – Little Bridge House

    Hospice support, including hospice breaks, emergency care, palliative care and end-of-life care, for children and young people living with life-limiting conditions. You can contact this service directly (self-refer), or a healthcare professional such as a Children’ Community Nurse may also offer a referral.

  • Palliative care

    The Palliative Care service provides nursing care and support for children and young people with life limiting and life threatening condition across Devon. You will need referral from a healthcare professional to access this service, for example paediatrician or Children’s Community Nurse.

  • Specialist wheelchair service

    Vranch House provides the specialist wheelchair service for children who have been assessed by a community therapist as needing specialist provision.
