- Confirmation of consultation with the emergency services
- Details of organisations, businesses or individuals consulted
Traffic management
- Details of traffic management used (road closure, rolling road closure, suspension of parking)
- Plan of diversion route
- Traffic management plan
Contractor qualification / accreditation details
- Name of Chapter 8 / CSAS accredited person / traffic management contractor
