DESCRIPTION:Please join the Dementia and End of Life NHSE SW teams to hear 
 about the importance of early discussion with carers and relative about pl
 anning end of life care for people with dementia. We have the Admiral Nurs
 e from Dorothy House Hospice in Bath talking on this subject. She will exp
 lain the Dorothy House model of care for dementia and will also cover symp
 tom recognition and pain control. Esther will start the session with a pre
 sentation and there will be plenty of time for questions and answers with 
 opportunity to share your experiences.\n\nThis is an MS Teams meeting and
