DESCRIPTION:We are pleased to announce the launch of a new Devon County Car
 e Home Forum starting on Tuesday 28 June\, 2-3pm.  An open invitation t
 o all care homes within the Devon County Council footprint to regularly me
 et with Devon’s Market Management Team\, CCG\, commissioners and other p
 rofessionals to share information on key topics of interest and concern\, 
 hear about and get involved in care home commissioning work and find out m
 ore about other key national and local care home priorities.\n\nThe first 
 agenda item we have agreed is Hospital Discharge\, with Sally Robins\, Qua
 lity Improvement Turnaround Director\, NHS Devon Clinical Commissioning Gr
 oup telling us more about the hospital discharge work she is leading on.\n
 \nWe will be working with DCHC (Devon Care Home Collaborative) to finalise
