Volunteers required for new Transition Pilot Project
Is your child 17+ with special educational needs and disabilities (SEND) and in the process of transitioning to adult services?
Devon County Council is currently looking for 6-10 families to work with us on a new pilot project, which is focusing on developing personalised care and support planning, including the use of personal budgets for those young people transitioning between children and adult services.
We recognise that this process can be very complex and does not always run as smoothly as we would like and so we want to work with families who currently receive a personal budget/direct payments to develop a process that is person centred and allows young people to transition into adult services seamlessly.
This will be a partnership approach between young people, families and professionals in order to develop a future way of working and it is intended to be creative and innovative – this means that we will be trying new things and some may not always work; but together we believe we can find solutions that allow greater choice and control and reduce the anxiety of the transition process.
