If you haven\u2019t received this within two working days, please check your junk or spam folders and then contact the SEND Statutory Team.\n\n \n<h2 class=\"wp-block-heading\">Help completing the form<\/h2>\n \n\nIt\u2019s okay if you can\u2019t complete a section of the form, just complete the sections that you can. We may still ask you for a lot of the information on the form to help with our decision-making.\n\n \n\nIf you need help to fill in the form, you can <a href=\"https:\/\/devonias.org.uk\/contact-us\/\" rel=\"noopener\">contact DiAS<\/a>.\n\n \n<h2 class=\"wp-block-heading\">Other ways to submit a request<\/h2>\n \n\nIf you don&#8217;t want to use the EHC Hub, you can also submit a request by:\n\n\n\n\n<ul class=\"wp-block-list\">\n \t<li>posting to SEND Statutory Team, Room L102, County Hall, Topsham Road, Exeter, EX2 4QD<\/li>\n \n \t<li>emailing\u00a0<a href=\"mailto:specialeducation@devon.gov.uk\">specialeducation@devon.gov.uk<\/a><\/li>\n \n \t<li>phoning\u00a0<a href=\"tel:01392380434\">01392 380434<\/a>\u00a0(option 1).
