between children and young people, their families and any supporting agencies.<\/p>\n<\/span><\/div>\r\n\r\n        <\/li><li class=\"dcc-step-nav__step js-step\" parent carers or professionals can request help, in other cases a referral may be needed.<\/p>\n<p>This can also be used where a child or young person is currently receiving support, but they have been identified as having an unmet need.<\/p>\n<p>Support may come from:<\/p>\n<ul>\n<li>Ophthalmology (via GP referral)<\/li>\n<li>Sensory and Physical SEN Team &#8211; <a href=\"https:\/\/www.devon.gov.uk\/support-schools-settings\/send\/request-for-access-to-send-support-guidance\/\">Request for access to SEND support<\/a><\/li>\n<li>GP or Paediatrician<\/li>\n<li>Disabled Children\u2019s Service &#8211; <a href=\"https:\/\/www.devon.gov.uk\/support-schools-settings\/send\/request-for-access-to-send-support-guidance\/\">Request for support<\/a><\/li>\n<li><a href=\"https:\/\/www.devon.gov.uk\/children-families-education\/send-local-offer\/understanding-send\/sensory-and-or-physical-needs\/sensory-impairments-useful-referral-contacts\/\">Sensory impairments useful referral contacts<\/a><\/li>\n<\/ul>\n<p>Individual teams will then apply their own decision making criteria before deciding whether to <strong>accept<\/strong>\u00a0or\u00a0<strong>decline<\/strong>\u00a0a request.<\/p>\n<p><strong>If a request is declined support may still be available from other sources &#8211; see Step produced.<\/p>\n<p>With consent, assessment plans and actions will be shared with those supporting the child or young person within education, health, and social care sectors.<\/p>\n<p>Where assessment is found to be more than the ordinarily available provision, a request for an <a href=\"https:\/\/www.devon.gov.uk\/children-families-education\/send-local-offer\/education-health-and-care-plans\/\">education, health and care (EHC) needs assessment<\/a> may be considered.<\/p>\n<\/span><\/div>\r\n\r\n        <\/li><li class=\"dcc-step-nav__step js-step\" id=\"monitoring-and-reviewing-6\">\r\n          <div class=\"dcc-step-nav__header js-toggle-panel\" data-position=\"6\">\r\n            <h2 class=\"dcc-step-nav__title\">\r\n              <span class=\"dcc-step-nav__circle dcc-step-nav__circle--number\">\r\n                <span individual service, to make sure that the planned outcomes continue to meet the assessed need(s).<\/p>\n<p>If support is no longer required, for example, identified outcomes are reviewed and achieved, from being a child to an adult. Planning the transition to adult services can start anytime from Year 9 or around age 14.<\/p>\n<p>See the\u00a0<a href=\"https:\/\/www.devon.gov.uk\/education-and-families\/send-local-offer\/preparing-for-adult-life-ages-14-25-years\/\">preparing for adulthood<\/a> section of the website for a range of information and advice, including our <a href=\"https:\/\/www.devon.gov.uk\/education-and-families\/send-local-offer\/preparing-for-adult-life-ages-14-25-years\/preparing-for-adulthood-transitions-guide\/\">transitions guide<\/a>\u00a0which describes the changes that young people can expect, and when these are likely to happen.<\/p>\n<p>The <a href=\"https:\/\/www.devon.gov.uk\/economy-enterprise-skills\/skills\/post-16-learning-participation\/\">Post-16 Learning Participation Team<\/a> (formerly CSW) work with young people aged 13 years and upwards, including those with sensory impairments, to help them plan for their future.
