You should use this protocol only when you have exhausted all usual escalation routes within your service &#8211; it is for exceptional situations only and does not replace standard business processes.<\/p>\n\n\n\n<p>The hope is that you will be able to resolve matters through the use of this protocol without needing recourse to the 16-17 forum. If, however, you do need to refer to the 16-17 forum, please <a href=\"https:\/\/forms.office.com\/pages\/responsepage.aspx?id=gzehjWjLP0S7S5l_d_1b-xLxTs4HGEhKrNvE2V4enOlURUlPOUJVSVgwTjhCVDRKOFk0MzZLTFdaVyQlQCN0PWcu&amp;route=shorturl\" rel=\"noopener\">complete the referral form<\/a>.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Next steps<\/h4>\n\n\n\n<p>Once you have completed the form, you will be invited to a meeting with appropriate professionals and senior officers to discuss how to improve the situation. These professionals may include experts from:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>children&#8217;s services<\/li>\n\n\n\n<li>adult services<\/li>\n\n\n\n<li>education services<\/li>\n\n\n\n<li>health services<\/li>\n\n\n\n<li>commissioners<\/li>\n\n\n\n<li>specialist areas<\/li>\n\n\n\n<li>relevant providers (if appropriate)<\/li>\n<\/ul>\n\n\n\n<p>Together, they will discuss options, work on finding solutions and make necessary improvements to support you to better help the young person.<\/p>\n\n\n\n<p>If the issue is a larger systemic gap, it will be discussed at the Transitions Governance Group, which represents the entire transitions system in Devon.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Protocol outcomes<\/h3>\n\n\n\n<p>The outcomes of this process should include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>the correct commissioning or case management pathway being clearly understood and agreed<\/li>\n\n\n\n<li>provision being in place by 18, or soon after the young person\u2019s 18th birthday<\/li>\n\n\n\n<li>a risk management plan being agreed upon<\/li>\n\n\n\n<li>barriers being unlocked.<\/li>\n\n\n\n<li>commissioning issues being resolved<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">The protocol<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">Introduction<\/h3>\n\n\n\n<p>This protocol provides a clear, step-by-step approach to planning and managing individual cases. It ensures that every action has a responsible person and that any gaps or issues are addressed.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Process overview<\/h3>\n\n\n\n<h4 class=\"wp-block-heading\">1. Person-led planning (Path or appropriate tool)<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Use a tool such as <a href=\"https:\/\/www.devon.gov.uk\/support-schools-settings\/send\/educational-psychology\/ep-services-support\/person-centred-planning\/\" target=\"_blank\" rel=\"noreferrer noopener\">Path<\/a> to outline the individual&#8217;s needs and goals.&nbsp;<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">2. Assign action owners<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Ensure each action item has a designated person responsible for it.<\/li>\n\n\n\n<li>If all actions have owners, proceed with the plan. If not, identify who should take responsibility.<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">3. Check involvement of services<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Determine if the action involves a service that is already engaged with the individual.<\/li>\n\n\n\n<li>If yes, speak to the line manager of the involved service to address the action item.<\/li>\n\n\n\n<li>If no, proceed to the next step to identify the appropriate service or individual to approach.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Referral process<\/h3>\n\n\n\n<h4 class=\"wp-block-heading\">4. Identify the right contact<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Assess if it&#8217;s clear who should take ownership of the action.<\/li>\n\n\n\n<li>If yes, make the referral to the identified service or person.<\/li>\n\n\n\n<li>If no, escalate the issue to the 16-17 transitions forum by <a href=\"https:\/\/forms.office.com\/pages\/responsepage.aspx?id=gzehjWjLP0S7S5l_d_1b-xLxTs4HGEhKrNvE2V4enOlURUlPOUJVSVgwTjhCVDRKOFk0MzZLTFdaVyQlQCN0PWcu&amp;route=shorturl\" rel=\"noopener\">completing the referral form<\/a>.<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">5. If you made a referral, was it successful?<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>If the referral is successful, update the plan.<\/li>\n\n\n\n<li>If not, decide who will escalate the issue to the service lead.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Services approach<\/h3>\n\n\n\n<h4 class=\"wp-block-heading\">6. Escalate to service lead<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Move to the next section to escalate the issue to the service lead for resolution.<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">7. Identify the issue type<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Determine if the issue is due to a gap in services or a capacity constraint.<\/li>\n\n\n\n<li>If it&#8217;s a gap, escalate the issue to the 16-17 transitions forum.<\/li>\n\n\n\n<li>If it&#8217;s a capacity issue, agree on when resources will be available and create a contingency plan.<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">8. Agreement and plan update<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>If an agreement is reached, update the plan.<\/li>\n\n\n\n<li>If not, escalate the issue to the next level.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Review and escalation<\/h3>\n\n\n\n<h4 class=\"wp-block-heading\">9. Monitor the plan<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Regularly review the plan&#8217;s effectiveness.<\/li>\n\n\n\n<li>If issues persist, escalate the case to the 16-17 transitions forum,<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">10. Clarify responsibilities<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>If unresolved, ask the 16-17 transitions forum to review the situation.&nbsp;<\/li>\n\n\n\n<li>If options are identified, discuss with the individual and update the plan accordingly.&nbsp;<\/li>\n\n\n\n<li>If no options are identified, the forum chair will clarify responsibilities.&nbsp;<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">11. Who is responsible?<\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>If clear, the 16-17 transitions forum will create an action plan. The service representative will find and present options.<\/li>\n\n\n\n<li>If unclear, the issue (without personal details) will go to the Transitions Governance Group, and if needed, to the SEND board to find the responsible service.<\/li>\n\n\n\n<li>After deciding, return to the 16-17 transitions forum for the service representative to find and present options.<\/li>\n\n\n\n<li>Once options are found, the escalation worker will discuss them with the individual and those involved to help make a decision. Update the person-led plan accordingly.<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">12.
