Your healthcare providers will start to talk to you what will happen then.<\/p>\n\n\n\n<p>Discussions will cover what will stay the same and what will be different. You will have the chance to talk about you, as well as your worries and concerns about your healthcare.<\/p>\n\n\n\n<p>You may find our <a href=\"https:\/\/www.devon.gov.uk\/children-families-education\/send-local-offer\/preparing-for-adult-life-ages-14-25-years\/health-and-wellbeing\/the-transition-to-adult-services\/\">transitions with health services<\/a> page useful.<\/p>\n\n\n\n<p>We\u2019ve also put together some general advice on <a href=\"https:\/\/www.devon.gov.uk\/children-families-education\/send-local-offer\/preparing-for-adult-life-ages-14-25-years\/health-and-wellbeing\/health-support-for-young-adults\/\">health support for young adults<\/a>.<\/p>\n\n\n\n<p>If you have significant or complex health needs, you will be invited to the&nbsp;<a href=\"https:\/\/www.devonscp.org.uk\/pathways-support-for-young-people-with-complex-health-needs\/\" target=\"_blank\" rel=\"noreferrer noopener\">Pathways Transition Clinic<\/a>. Here, a handover will be done between your paediatrician and the adult consultant physician.
