{"id":20401,"date":"2024-09-10T14:27:52","date_gmt":"2024-09-10T13:27:52","guid":{"rendered":"https:\/\/www.devon.gov.uk\/children-families-education\/?page_id=20401"},"modified":"2026-03-20T15:06:07","modified_gmt":"2026-03-20T15:06:07","slug":"providing-your-evidence-to-support-the-assessment","status":"publish","type":"page","link":"https:\/\/www.devon.gov.uk\/children-families-education\/send-local-offer\/education-health-and-care-plans\/providing-your-evidence-to-support-the-assessment\/","title":{"rendered":"Providing your evidence to support the assessment"},"content":{"rendered":"\n<h2 class=\"wp-block-heading\">What we will need from you<\/h2>\n\n\n\n<p>When providing evidence for an EHC needs assessment, you should consider including the following:<\/p>\n\n\n\n<p><strong>An account of your child\u2019s needs<\/strong>: You can write a detailed account of your child\u2019s difficulties, the impact on your daily life and education, and any support you believe your child needs.<\/p>\n\n\n\n<p><strong>Reports from your child\u2019s place of education<\/strong>: This could include observations, assessments, and any interventions that have been tried.<\/p>\n\n\n\n<p><strong>Doctors\u2019 assessments of your child<\/strong>: If your child has seen any medical professionals for their needs, any resulting reports or assessments can be very helpful.<\/p>\n\n\n\n<p><strong>Evidence of your child\u2019s physical, emotional, and social development<\/strong>: This can include reports from clinicians and other health professionals, as well as information on what has been done to meet these needs by other agencies.<\/p>\n\n\n\n<p><strong>Information on your child&#8217;s progress<\/strong>: Evidence that, despite relevant and purposeful action taken by the early years provider, school, or post-16 institution to identify, assess, and meet the special educational needs of your child, they have not made expected progress.<\/p>\n\n\n\n<p>It&#8217;s important to provide a comprehensive picture of your child&#8217;s needs and the support they require. We will use this evidence along with all the other advice gathered to determine whether an EHCP should be made for your child.<\/p>\n\n\n\n<p>The DiAS website provides lots of helpful advice about <a href=\"https:\/\/devonias.org.uk\/information\/parent-and-child-views\/\" rel=\"noopener\">getting your child\u2019s views as well as what to include in your own<\/a>. If you need more help to understand the process, <a href=\"https:\/\/devonias.org.uk\/contact-us\/\" rel=\"noopener\">contact DiAS<\/a>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Submitting your evidence on the EHC Hub<\/h2>\n\n\n\n<p>There are two areas for you to record your views as well as those of your child. \u2018About Me\u2019 and \u2018Parent\/Carer Views\u2019 are multimedia spaces where you can type text, add photographs or short video clips.
