Use the Devon EHC Hub<\/h2>\n\n\n\n

Although you don’t have to use the EHC Hub to submit a request, we recommend that you do so as it’s the easiest way to make sure you submit all the information we need.<\/p>\n\n\n\n

The Hub is the central online resource for seeing exactly what\u2019s happening and what information has been provided by those involved in the assessment.<\/p>\n\n\n\n

You\u2019ll also be able to see information around timescales so that you can keep track of when the main parts of the process should be started and finished.<\/p>\n\n\n\n

Once your request has been processed, you will receive an email asking you to register on the Hub<\/a>.<\/p>\n\n\n\n

It\u2019s okay if you can\u2019t complete a section of the form, just complete the sections that you can. We may still ask you for a lot of the information on the form to help with our decision-making. If you need help to fill in the form, you can ask DiAS to help you<\/a>.<\/p>\n\n\n\n
