Schools will have their own system for transition and these should keep the needs of the child and family at the heart of all they do.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A transition meeting should be arranged so that you and early years setting staff, as well as any other professionals who work with your child, can discuss their strengths, needs and required support. Notes from the meeting should be used to help with transition and to ensure your child has all the help they need to settle into their placement.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If your child has an EHCP in place, a review should be held as part of the planning process and to identify the school your child will attend. The review should happen at the beginning of their last nursery year and will be held by the setting they are in.
