{"id":44477,"date":"2025-06-20T11:54:14","date_gmt":"2025-06-20T10:54:14","guid":{"rendered":"https:\/\/www.devon.gov.uk\/adult-social-care\/?page_id=44477"},"modified":"2025-10-24T12:20:15","modified_gmt":"2025-10-24T11:20:15","slug":"reaching-for-independence-service","status":"publish","type":"page","link":"https:\/\/www.devon.gov.uk\/adult-social-care\/independent-living\/reaching-for-independence-service\/","title":{"rendered":"Reaching for Independence service"},"content":{"rendered":"\n<p>Reaching for Independence (RFI) is Devon County Council\u2019s service that helps support people to develop life skills and promote their independence.<\/p>\n\n\n\n<p>This service can be offered in your community or at one of our venues around Devon.<\/p>\n\n\n\n<p>The service offers intensive short-term intervention support focussed on helping people to:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>develop or increase their skills (or both) in things such as money management, cooking and household management<\/li>\n\n\n\n<li>take up new opportunities such as starting paid employment<\/li>\n\n\n\n<li>gain the skills and confidence to move to other options, such as community facilities and services, independent or supported living<\/li>\n<\/ul>\n\n\n\n<p>The service supports people to achieve the goals that are most important to them, helping them to become more independent and lead a fulfilling life. Where appropriate, technology or equipment (or both) will be used to help encourage this independence.<\/p>\n\n\n\n<p>Although support provided by the service is short-term, it is not time limited. The time spent by the service supporting a person is decided by the needs of the individual, the specific goals being worked on, and the progress being made by the person towards achieving those goals.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Who this service is for<\/h2>\n\n\n\n<p>The service is available to people:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>aged 17 years and over, where it has been identified that they would have eligible or likely eligible social care needs<\/li>\n\n\n\n<li>known to adult social care who have eligible social care needs and are currently receiving care and support services to meet those needs<\/li>\n\n\n\n<li>new to adult social care who have had eligible social care needs identified by an assessment, or following initial contact it is thought<br>they would have eligible social care needs at that time<\/li>\n\n\n\n<li>with a range of needs, including those with the most complex needs<\/li>\n\n\n\n<li>who, as a result of a formal carer&#8217;s assessment, have been identified as having an eligible carer need, and may benefit from the enabling support offered by the service<\/li>\n<\/ul>\n\n\n\n<p>The service is also available to anyone else who has been assessed as eligible for enabling support.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What to do next<\/h2>\n\n\n\n<p>For anyone aged 17, you can contact the service by <a href=\"https:\/\/www.devon.gov.uk\/children-families-education\/send-local-offer\/preparing-for-adult-life-ages-14-25-years\/moving-from-childrens-social-care-to-adult-social-care\/\">submitting a referral to the Transitions Adult Social Care team<\/a>.<\/p>\n\n\n\n<p>For anyone aged 18 and above, please call Adult Social Care on <a href=\"tel:03451551007\">0345 155 1007<\/a>.
