Please could you provide me with the following information regarding the number of referrals to children’s services made by teachers/schools regarding pupils who had self-harmed* from the beginning of January 2013 to the end of December 2018:
* The number of referrals
* The number of referrals made per year
* Details of each referral including, but not limited to:
o The date
o The age and gender of the child who was referred
o What action was taken by children’s services
*Mental Health Foundation (2003) definition of self-harm:
* Self-harm is self-harm without suicidal intent, resulting in non-fatal injury
* Attempted suicide is self-harm with intent to take like, resulting in non-fatal injury
* Suicide is self-harm resulting in death
