
Young people with special needs and their families face a unique set of challenges throughout the transition process, and have to make important decisions and choices.
These pages will help you to think about different issues and options, providing information, as well as signposts to other sources of knowledge and advice, including some direct links to other sites. There will be some links which will give you details about services specific to the area that you live in, as well as more general information.
We hope that this holistic approach will provide you with enough detail to begin thinking about the issues involved, but we will also signpost you in the direction of other people who can help.

The Devon Transitions Protocol was agreed in May 2004, since then a lot of practical work has been done on its implementation. It is being introduced gradually starting with a few young people in year 9 in September 2005. During the Autumn of 2005 most of the practitioners concerned will be trained to use the Protocol, and after that more and more young people will be following the Protocol in their transition process.
However most of the information contained in these pages is still relevant to young people who have already started the transition process and are not following the protocol.
