This film 'The Age of Opportunity in Devon - the Link Age Plus Difference' explains the work of Link Age Plus (LAP). LAP was intended to pilot ways of providing fully integrated services for older people. It was a Government5 initiative to tackle social exclusion among people aged 50 plus, particularly those who are 'harder to reach' such as frailer people and people from minority communities.

As our population ages, an increasing number of people face the social isolation and loneliness which can frequently follow common later life events such as bereavement, illness, disability or becoming a carer. This film looks at the way community mentoring can help isolated individuals achieve a permanent improvement in the day-to-day lives. It follows the case studies of people who have undergone community mentoring and also features a GP's perspective.
