Part 3: Section 3.2

No more than 16% of Children Looked-after have three or more placements in any Council Stability of placements of Children Looked-after (PAF A1, BVPI 49) Point 1. Ensuring stability of care for children whether at home or Looked-after.
By 2002 reduce by 10% the proportion of children who are re-registered on the Child Protection Register from a baseline for the year ending March 1997. Re-registrations on the Child Protection Register (PAF A3).

Point 2. Ensuring children are protected from abuse.

To carry out 100% of reviews of child protection cases that should be carried out in any one year Reviews of child protection cases (PAF C20) Point 2. Ensuring children are protected from abuse.
Improve the educational attainment of children and young people in care by increasing from 6 per cent in 1998 to 15 per cent in 2004 the proportion of children leaving care aged 16 and over with 5 GCSEs at grade A*-C and maintain this level up to 2006. Educational Qualifications of Children Looked-after (PAF A2, BVPI 50) Point 4. Promote educational, health and social care of Looked-after Children.
Reduce by 2004 the proportion of children aged 10-17 and Looked-after continuously for at least a year who have received a final warning or conviction, by one third from September 2000 position. (Reduce the proportion from the 2000 national figure of 10.8 per cent to a local maximum of 7.2 per cent) Maintain this reduction up to 2006. Improve the level of employment, training or education amongst Care Leavers aged 19 so that levels for this group are at least 75% of those achieved by all young people of the same age in their area by March 2004 and maintain this level up to 2006 Employment, education and training for Care Leavers (PAF A4)

Point 5. Ensure that Care Leavers are not isolated and participate socially and economically as citizens.

Older People

 

Part 3: Section 3.8

By December 2004 all assessments of older people will begin with 48 hours of first contact with Social Services and will be completed within four weeks, (with 70% within two weeks); following assessment, all Social Services will be provided within four weeks, with 70% in 2 wks.

Point 2: Person Centred Care.
Improve the quality of life and independence of older people so that they can live at home wherever possible, by increasing by March 2006 the number of those supported intensively to live at home to 30% of the total being supported by Social Services at home or in residential care.

Medium Term Strategy.
By April 2004 all general hospitals caring for people with stroke to have a specialised stroke services and all health and social care systems to have established an integrated falls service by 2005. No PAF indicators Point 5: Joint Agency Stroke Group.
All community equipment for older people (aids and minor adaptations) will be provided by Social Services within seven working days. % of items of equipment delivered within 7 working days (D54) Point 6: Older People.
Ensure that by April 2004 protocols are in place across all health and social care systems for the care and management of older people with mental health problems No PAF indicators currently Point 7; Older People with mental health difficulties
Promote independence by reducing nationally the per capita rate of growth in emergency admissions of people aged over 75 to an annual average of 3% over the five years up to 2002-03, compared with an annual average rate of 3.5% over the last five years.

Part 3: Section (Various)

Direct Payments give recipients control over their lives, increase opportunities for independence, social inclusion and self-esteem. From April 2003 councils have a duty to make direct payments where individuals consent to and can manage them.
