Modernisation Programme
Modernisation Programme
Adult & Community Services is modernising and becoming more personalised. We are updating our services to provide more choice, flexibility and independence for people through services tailored to meet their needs. We are doing this in partnership with the NHS forming fully integrated teams wherever possible
There are a number of broad themes and key developments.
Modernising our 'Care Pathway', - that is how people contact us, the way we assess their needs and arrange services. From the first time you contact us on our single telephone number 0845 155 1007 your request will be dealt with promptly and efficiently. You can see the new ways of working in action by watching our DVDs or reading an evaluation (274KB - pdf help) based on findings from our pilot areas With our new Personal Brokerage service we can now offer packages of care tailored to meet the needs of the individual.
Externalisation - Changing the way we provide services, that is residential care, day services which take place within our residential homes and domiciliary care services. This will create an exciting 10 year development programme to increase the volume and range of services available to people all over Devon
Improving the Disability Equipment and Mobility Service (46KB - pdf help) Reducing waiting lists by meeting requests for simple equipment more quickly and efficiently, allowing specialist staff to concentrate on people with complex needs Read the evaluation (386KB - pdf help) from our pilots.
Integration - We are joining up our services with the NHS and other partners in order to offer a seamless service for the public wherever possible. My Life, My Choice is a two year, government funded, partnership programme which is helping to make this happen.
Mental health and learning disability services are provided by the Devon Partnership NHS Trust. We are providing more services at home or close to home; reducing reliance on hospital-based care; improving quick and easy access to services and focusing everything we do on recovery. Helping people with learning disabilities to lead more fulfilling lives. A new publication Next Steps sets out our plans
In a series of articles published in the national magazine, 'Community Care',.David Johnstone, Executive Director, Adult & Community Services explains our new ways of working:
12 June 2008 - Devon's plans to change care management,
26 June 2008 - Personalisation: the rapid equipment service in Devon
08 July 2008 - Integration in Devon
31 July 2008 - Letter from David Johnstone, Executive Director, to the Editor of Community Care in response to anonymous comments printed in the magazine.
“If I had to sum up in a sentence the aim of our proposals it would be to provide the kind of care I think any one of us would want for our own family. By that I mean ensuring that the people who need our help have the chance to enjoy more independence, supported by flexible services tailored to their needs” said David Johnstone
Modernising will mean reinvestment from old-style services into new ones. This will affect the full range of social care activity, which is increasingly carried out in partnership with the NHS and other agencies.
