Devon & Torbay Development Project for Older People's Mental Health
The Devon Older People's Strategic Partnership (which has commissioned this project), is the lead forum for developing older people's health and social care across Devon, excluding Torbay and Plymouth. The Partnership includes members from the NHS, Social Services, the voluntary sector and representatives from older people's forums.
Project Aims:
- To focus on developing care pathways that will enable a person-centred rather than a service-centred approach.
- To develop care pathways which follow people's journeys through services, that promote 'seamless' experiences for older people with mental health needs and their carers.
- To look at how individual services provided by different organisations can work together.
- To consider how future developments can be planned for and implemented effectively.
- To evaluate the effectiveness of all services, identifying strengths and possible areas for improvement
- To consider all service providers when developing pathways.
