Devon County Council has a duty to continue to 'advise, assist and befriend’ young people leaving our care until the age of 21.
All young people who have been in care in Devon will be allocated to the appropriate team when they are 15-16 years old. Their current care manager will arrange this.
While the teams work closely with young people on a one-to-one basis, we also work with other partner agencies such as housing, YMCAs, careers services, Young Devon, youth offending teams and others to provide a holistic service to young people.
This support is planned before the young person leaves care, and will be arranged by the young person's current care manager.
