About Devon Partnership NHS Trust

The Trust provides mental health and learning disability services to the population of Devon with the exception of the city of Plymouth.

We recognise the importance for service users and carers of improving their access to services. For this reason our adult mental health and learning disability services are provided in partnership with the two Social Services Departments of Devon and Torbay. This means that our staff work in single teams under one management arrangement.

Care Management in Devon

Care management is part of the NHS Care Programme Approach, assuring that people with mental health issues are assessed in an holistic way, addressing health and social care needs equally.

Integrated services

Community Mental Health Teams (CMHTs)

There are Community Mental Health Teams across the County that provide assessment, community care services and community support for people managing their day-to-day mental health needs.

Crisis services

There are separate teams across Devon who provide intervention at times of crisis, to enable people to remain in their own home.

Assertive outreach

Assertive outreach teams work with people who have difficulty in engaging with the service and have a high level of need.

Day opportunities

There are a range of educational, work and therapeutic services available across the county for rehabilitation and enabling people to move forward in their lives.

Service user and care involvement

By assessing carers’ needs we can offer them support to continue wtih their role as carers.

Both service users and carers are actively involved in all aspects of the service through a number of initiatives and networks.

Inpatient services

Acute inpatient care is provided in a number of specialist sites to serve the local population.

Standards for Learning and Development

Approved social work training and mandatory refresher training has been established for some years.
