Delivering the Outcome in 2007/08 – Front Line Services To achieve this outcome we will: |
Responsible |
|---|---|
| Note: Many of the objectives for this outcome are being delivered via objectives detailed in the previous outcome areas. | |
| Increase the proportion of older people receiving Direct Payments to traditional personal care services and reduce the number of older people admitted permanently to residential and nursing care. In doing so, we will ensure that service users feel more in control of their daily lives, feel safer at home and have as much contact with others as they want. We will ensure that service providers do what service users want and generally increase service user satisfaction with services. | Assistant Director (Change Programme) |
| Ensure that as we modernise business processes we maintain a focus on an individual’s dignity through diligent attention to practice, with particular focus on choice, individualised and independent approaches to care in all cases. | All ADs |
| Implementing the recommendations arising from the investigations into services for people with learning disabilities at Cornwall Partnership Trust, and the London Borough of Sutton and Merton. The Devon response is shaped within an Action Plan and a joint review of services in Devon with Devon Partnership Trust and Devon Primary Care Trust. | Assistant Director (LD) |
| Support the NHS in the implementation of “End of Life” gold standards across care and housing support. | All ADs |
| Ensure that we maintain a focus on an individual’s dignity while implementing the Learning Disability Recovery Programme by taking account of individualised care needs in all cases. | Assistant Director (LD) |
