Delivering the Outcome in 2007/08 – Front Line Services

To achieve this outcome we will:

Responsible
Increased use of the Home Improvement Agency for advice about home insulation, fuel poverty and energy efficiency (especially in the most disadvantaged areas) to improve the health and home environments of more people. Assistant Director (SP&C)
Ensure assessments, care plans and reviews identify opportunities for employment, learning and training, to increase the number of economically active vulnerable adults, older people and their carers. All ADs
Implement the new carers’ assessment and simplify access to Jobcentre Plus and Next Step within care management, to improve our response to carers’ employment and training needs. All ADs
Ensure a rights-based approach for people with complex health and social care needs. All ADs
Increase applications to the Independent Living Fund for all eligible people enabling more people to buy their own care and support in the way they want it. All ADs

Delivering the Outcome in 2007/08 – Planning and Support Services

We will support implementation by:

Commissioning Carers Link to undertake a comprehensive welfare benefits check for all their members on joining and on annual renewal of membership. Assistant Director (SP&C)

Working with other agencies and organisations to facilitate improved access to employment for people with a learning disability. Assistant Director (LD)
