Delivering the Outcome in 2007/08 – Front Line Services

To achieve this outcome we will:

Responsible

Simplify access to assessment and support and address individuals’ needs at an early stage by making preventative advice and support more widely available. This will be achieved by bringing together the health and social care workforce into integrated teams.

The integrated teams will address housing support, health and social care needs locally and prevent unnecessary admissions to hospital and support people to go home from hospital in a timely way and appropriate support.

Improve access to nail cutting and footcare by developing a new service in partnership with the Devon PCT and the voluntary sector. This will enable 4,800 older people to receive the service over the next 2 years. Assistant Director (SP&C / OP/PD)
Promote independence, build confidence and a sense of well being amongst older people though a new, locally based, time limited support service arranged in partnership with the Devon Primary Care Trust and the voluntary sector. The service known as the Community Mentor Service will support 150 older people in the next 2 years. Assistant Director (SP&C / OP/PD)
Achieve equitable and accessible services to adults with a learning disability, including promoting easy to understand information by further developing the Health Facilitator roles and support mainstream services. Assistant Director Learning Disability (LD)
Commence implementation of the ‘Greenlight Strategy’ within mental health services for supporting people with a learning disability who experience mental health problems. Director of Care Devon Partnership Trust Mental Health (DPTMH)
Increase carers awareness of, and access to, the wide range of services available through Carer’s Link. This voluntary sector service can arrange flexible breaks, provide small amounts of funding for individual use to support carers have a break (452 carers used the service in 06/7), arrange respite services in people’s homes (Take A Break Service),  and undertake carers benefits checks (163 completed in 06/07). Improve the experience of young people and their parents when their support is transferred from Children to Adult social care. The first phase of this will be to implement an agreed transition / transfer protocols with young people and their families. Assistant Director (PD / LD / DPTMH)

Delivering the Outcome in 2007/08 – Planning and Support Services

We will support implementation by:

Reducing duplication for users and carers and enable front line staff to effectively meet their needs by  integrating  management, finance, business support and performance monitoring functions between DCC, Devon PCT & Devon Partnership Trust. Assistant Director Finance & Business Resources (F&BR)
Tackling the complex problems that impact on individuals health and wellbeing e.g. worklessness and vulnerability, by setting up a Health and Well Being Partnership that goes beyond the organisational boundaries of adult health and social care. Director / Assistant Director (SP&C)
Enabling the right care, at the right time and in the right place based on understanding of best practice, what people say is important to them and the Devon health needs assessment. The County council and the primary care trust will use this information to set out a joint commissioning strategy to develop and improve services. Assistant Director (SP&C)
Developing the processes, policies, protocols, workforce planning and learning and development to support the introduction of integrated health and social care workforce. Assistant Director (F&BR)
