Care Management
Support Planning
What is a care plan?
“Care planning should be responsive to – but not prejudiced against – the age, living circumstances, geographic location, disabilities, gender, culture, faith, personal relationships and lifestyle choices of service users. Care planning should build on the strengths and abilities of individuals and the part they can play in addressing their needs. It should address external or environmental factors that have caused the need to arise, or will hamper the resolutions of need if not addressed.” Guidance on the Single Assessment Process, Department of Health, 2002. |
