November 2011 - Hospital Discharge Project
A presentation on this project was given to the Southern Locality Forum although it is relevant to all provider areas. Sharon Goldsworthy, the project leader, also suggested that care homes should complete the South West Ambulance Form when patients are being admitted to hospital.
