Hospital Discharge Service RD&E (Wonford)
The Hospital Discharge team based at the Royal Devon & Exeter is a multi-agency, multi-disciplinary team which deals with discharges of patients from the RD&E and local Community Hospitals. The team includes Social Workers, Community Care Workers, Occupational Therapists, and Nurses with responsibilities for discharge to intermediate care services across the surrounding localities.
The team manages a high turnover of referrals for patients requiring a range of community and primary care services.
Referrals are made to the team predominately by ward staff and the Acute Trust Occupational Therapy service. If a patient does not have a Social Services personal care manager within the relevant locality then the team will take on the referral. The case is transferred back to the relevant locality at approximately eight weeks, or when the care package is stable.
