Information for Providers

Information for providers of Carer's Health & Well-Being Checks and partner organisations.

Provider meetings were held on 7 & 8 December 2010.  Here are some of the key concerns that were raised image - PDF icon (23KB - pdf help) and ideas for addressing them.

An interim research report image - PDF icon (916KB - pdf help) was produced in September 2010 by the University of Plymouth, this presentation shows some of their initial findings.

We would like to help you to identify carers and promote checks – if you have room in your reception areas and would like to borrow one of these ten sets of smart pull-up displays, on short term loan (One or two weeks), please telephone the office on 01392 825756 or email h.spackman@westbankfriends.org to order fliers and posters.

They will also be on loan to libraries, supermarkets and other venues where carers may see them. Participating practices have leaflets already but we have designed a more general flier image - PDF icon (2.65MB - pdf help) to go with the new displays and a poster image - PDF icon (2.31MB - pdf help) for where space is too limited for a large display.

Appointment letters - Some practices are sending invitations to carers to book a check, and adding other local information.  Here is one completed by the Moorlands practices image - PDF icon (184KB - pdf help) as an example and a word template that you can adapt for your own use

The booklet, leaflets, posters and other material were designed by Stephanie Cheesman - ACS Public Information & Communication Team and were specially praised by the Department of Health and other participating local authorities. Many people contributed to the final content including Carers, Public Health, Drug & Alcohol Action Team (DAAT) and GP practice staff.

We are especially proud that Stephanie won a 'Cream of Devon' staff achievement award for this work after being nominated by Clare Cotter, Programme Lead.
