We are doing this in partnership with the NHS forming fully integrated teams wherever possible

Externalisation - Changing the way we provide services, that is residential care, day services which take place within our residential homes and domiciliary care services. This will create an exciting 10 year development programme to increase the volume and range of services available to people all over Devon

Integration - We are joining up our services with the NHS and other partners in order to offer a seamless service for the public wherever possible.
Mental health and learning disability services are provided by theDevon Partnership NHS Trust. We are providing more services at home or close to home; reducing reliance on hospital-based care; improving quick and easy access to services and focusing everything we do on recovery. Helping people with learning disabilities to lead more fulfilling lives.

Modernising will mean reinvestment from old-style services into new ones. This will affect the full range of social care activity, which is increasingly carried out in partnership with the NHS and other agencies My Life, My Choice is a two year, government funded, partnership programme which will help start to make this happen
