1. A review should take place using the SS3
2. If services need to be ongoing then the appropriate Practice Managers from the two teams should agree when and how the case will be transferred.
3. The care manager should record the agreement on a History Report Form SS2a or a Monitoring Form SS6.
4. Business Support will be responsible through use of Form SS13 for amending records and CareFirst and sending the relevant paperwork to the new team. More detailed information about this is contained within Devon Social Services Adult Care Management / CareFirst manual.
5. The receiving team are responsible for the costs of providing the care package from an agreed date.
6. There is no budgetary transfer of funds from one team to another. However the two teams must consider what action, if any, needs to be taken if the service user has accrued a debt due to the non payments of bills.
