Community Enabling Service
WHAT IS THE COMMUNITY ENABLING SERVICE ?
The Community Enabling Support Service encourages people who have a disability to:
- participate freely within the community
- increase their own independence
- achieve goals agreed by themselves with the support of their families, carer and Care Manager
Clients will have been identified as having the potential to take more responsibility for their own lives and to decrease their dependency on other people.
They will have expressed an interest to develop their skills in various areas such as:
- home management
- budgeting and money management
- social and leisure activities
- personal care and health maintenance
- education, such as IT training
- employment
Clients must be prepared to:
- enter into a process of possible significant changes in lifestyle
- take risks or make mistakes as part of a positive learning process
- develop understanding of their strengths, limitations and potential
What can the service provide?
The Community Enabling Service provides:
- an intensive, short term service within agreed timescales
- specialist assessment of an individual's abilities, interests and aspirations
- supported programmes that are flexible and individually planned
- practical help and encouragement in use of community facilities
How is someone referred to the Community Enabling Service?
- Clients can only receive the Community Enabling Service as part of their care plan agreed with their care manager.
- Care manager refers client to service.
