Over the past few years we have been 'modernising' and 'personalising' Adult & Community Services. We have been updating our services to provide more choice, flexibility and independence for people through services tailored to meet their needs. We have continued to do this in partnership with the NHS forming fully integrated teams wherever possible.
This web page is an archive of the development over the past few years.
Currently our transformation of social care services, with updates on the implementation of key personalisation changes can be found on our Putting People First web pages, (from 2010- current).
There are a number of broad themes and key developments.
Modernising our 'Care Pathway', - that is how people contact us, the way we assess their needs and arrange services. From the first time you contact us on our single telephone number 0845 155 1007 your request will be dealt with promptly and efficiently.

Changing the way we provide services, This will create an exciting 10 year development programme to increase the volume and range of services available to people all over Devon
Improving the Disability Equipment and Mobility Service
(46KB - pdf help) Reducing waiting lists by meeting requests for simple equipment more quickly and efficiently, allowing specialist staff to concentrate on people with complex needs
Integration - We are joining up our services with the NHS and other partners in order to offer a seamless service for the public wherever possible. My Life, My Choice was a two year, government funded, partnership programme which helped to make this happen.
Mental health and learning disability services are provided by the Devon Partnership NHS Trust. We are providing more services at home or close to home; reducing reliance on hospital-based care; improving quick and easy access to services and focusing everything we do on recovery. Helping people with learning disabilities to lead more fulfilling lives. A new publication Next Steps sets out our plans
