1. The name of the parent authority that you work for ('Devon County Council' or 'Plymouth City Council' for example).
2-4. Your title (Mr/Mrs/Miss/Dr etc), initials and surname.
5-10. Your home address.
11-14. Your forename(s), surname, date of birth and NI number.
15. The date you left your employment (or opted-out of the pension scheme) while working for an authority who's members contribute to the Devon County Council Pension Fund.
16. The date from which your pension benefits are payable (see 19). The normal retirement date for men and women in the LGPS is their 65th birthday.
17. This number is taken from the Retail Price Index figure of the preceding year (e.g. statements produced in March 2005 used an RPI figure from September 2004). This percentage is then applied to the pension for every year up to the members normal retirement date.
18. The financial year in which the most recent pensions increase has been applied.
19. The estimated value of the annual pension payable on the members 65th birthday.
20. Same as 17, except the figure shows the portion of the lump sum retirement grant relating to pensions increase.
21. The estimated value of the lump sum retirement grant payable on the members 65th birthday.
22. Same as 17, except the figure shows the portion of the spouse/civil partner's pension relating to pensions increase.
23. The estimated value of the spouse/civil partner's pension payable on the event of the members death.
24. The total percentage of pensions increase used in the calculations.
25. The employer you worked for during the period of service in question. This does not necessarily have to be another local government employer; you are able to transfer service from a wide range of public and private sector employments, as well as personal pension plans (in which case, the name of the company you have the plan with will be shown here).
26-27. Dates showing the period of service you were with your former employer/company. If you changed your hours at any time during your previous employments, you may well have several service dates relating to the same employment (most notably in previous local government employment). If you transferred-in a personal pension plan, the dates in this section will always be shown as the day immediately preceding the start of your current employment.
