Intervention Team:
Aim:
To develop a service which will provide support for young people and their families who are experiencing difficulties which are likely to lead to a breakdown in the family situation and foster placements.
Objectives:
- To reduce the number of emergency admissions into residential and fostering services.
- To provide an alternative option to Care Managers when faced with potential family breakdown.
- For the services to be responsive and easily accessed.
- To enhance the service which can be offered by a Care Manager in supporting a family.
- To be a service which has excellent links and working relationships with other community resources for young people.
Criteria For Access To Service:
- Age: 8-16 years. In occasional circumstances the age range can be reduced to 8 years old, if agreed by Intervention Manager and Practice Manager.
- Initial or core assessment indicates family or placement breakdown to a point that alternative accommodation is likely to be required. Social work assistance is by itself insufficient to have an impact on the problems.
- Request would be authorised by Team Manager or Practice Manager and a Care Manager must be appointed to the case as named worker.
Provision of Care Bank Services - for Spot Purchasing
