Intervention Team:

Aim:

To develop a service which will provide support for young people and their families who are experiencing difficulties which are likely to lead to a breakdown in the family situation and foster placements.

Objectives:

  • To reduce the number of emergency admissions into residential and fostering services.
  • To provide an alternative option to Care Managers when faced with potential family breakdown.
  • For the services to be responsive and easily accessed.
  • To enhance the service which can be offered by a Care Manager in supporting a family.
  • To be a service which has excellent links and working relationships with other community resources for young people.

Criteria For Access To Service:

  • Age: 8-16 years. In occasional circumstances the age range can be reduced to 8 years old, if agreed by Intervention Manager and Practice Manager.
  • Initial or core assessment indicates family or placement breakdown to a point that alternative accommodation is likely to be required. Social work assistance is by itself insufficient to have an impact on the problems.
  • Request would be authorised by Team Manager or Practice Manager and a Care Manager must be appointed to the case as named worker.

Provision of Care Bank Services - for Spot Purchasing

Referral Process for both Intervention and Care Bank Services:

  1. Team Leader/Team Manager will be happy to discuss a case with the Care Manager by phone prior to referral.
  2. Referral forms have been supplied to all districts and can be faxed or posted to the Intervention Team at Brookhayes, Pilton Lane, Pinhoe, Exeter EX1 3RA.
  3. The Intervention Team will contact the Care Manager on receipt of the Referral Form to arrange a meeting to discuss the case.
