Who Can Help?

There are people who you can talk to if you need more help to understand what transition is all about, and about different aspects of transition.  Some of these people you will know already.

Key worker/ Care Manager

Your key worker or care manager will be able to talk to you about all aspects of transition, and help you to decide who else you might want to get information from.   It is usually this person who is responsible for coordinating all the information from everyone else involved into an integrated care plan.

Connexions Personal Advisor

Your Connexions personal advisor is another important source of information.  They can advise you on all aspects of further education, vocational training and/or work opportunities, including applying for funding should you need it.

Your school

There are probably members of staff who can offer you support and information during the transition period.  They have links with local colleges, and usually with companies who provide work experience placements.  Ask questions at the Transition review -make a list before you go if that helps – and if there is not time to get all the answers you need ask to make another appointment.

Advice / Information Centres

There are centres all over Devon that will be able to signpost you to many different services that are available to young people.  This could include counselling, housing, money advice, social and leisure opportunities, employment and education.

Citizens Advice Bureau

Staff at CAB offices aim to provide a comprehensive advice service to the local community: it is free, impartial, confidential and independent.  They can offer advice on:

  • Debt and welfare benefits
  • Family issues
  • Housing matters (including disputes and nuisance neighbours)
  • Employment law
  • Consumer rights
  • Any area concerned with civil law.

Department of Work and Pensions

The Department for Work and Pensions (DWP) is responsible for the Government's welfare reform agenda.  Its aim is to promote opportunity and independence for all.  It delivers support and advice through a modern network of services to people of working age, employers, pensioners, families and children, and disabled people.
